Design and initial results of a programme for routine standardised longitudinal follow-up after congenital heart surgery

Background
With improvements in early survival following congenital heart surgery, it has become increasingly important to understand longer-term outcomes; however, routine collection of these data is challenging and remains very limited. We describe the development and initial results of a collaborative programme incorporating standardised longitudinal follow-up into usual care at the Children’s Hospital of Philadelphia (CHOP) and University of Michigan (UM).
MethodsWe included children undergoing benchmark operations of the Society of Thoracic Surgeons. Considerations regarding personnel, patient/parent engagement, funding, regulatory issues, and annual data collection are described, and initial follow-up rates are reported.
ResultsThe present analysis included 1737 eligible patients undergoing surgery at CHOP from January 2007 to December 2014 and 887 UM patients from January 2010 to December 2014. Overall, follow-up data, of any type, were obtained from 90.8% of patients at CHOP (median follow-up 4.3 years, 92.2% survival) and 98.3% at UM (median follow-up 2.8 years, 92.7% survival), with similar rates across operations and institutions. Most patients lost to follow-up at CHOP had undergone surgery before 2010. Standardised questionnaires assessing burden of disease/quality of life were completed by 80.2% (CHOP) and 78.4% (UM) via phone follow-up. In subsequent pilot testing of an automated e-mail system, 53.4% of eligible patients completed the follow-up questionnaire through this system.
ConclusionsStandardised follow-up data can be obtained on the majority of children undergoing benchmark operations. Ongoing efforts to support automated electronic systems and integration with registry data may reduce resource needs, facilitate expansion across centres, and support multi-centre efforts to understand and improve long-term outcomes in this population.

The subsequent contraceptive behaviour following reported side effects in users of oral contraceptives in the southern region of Brazil is examined in relation to discontinuation of pill use, changing to other methods, termination of contraceptive use, the role of the physician in influencing a woman's decision to discontinue pill use, and discontinuation according to the type of problem experienced.
In 2904 currently married women, aged 15–44, almost 75% reported that they had used the pill at some time, and of these 45.6% were still doing so. Women who reported problems with the pill were less likely to be current users (25%) than the women who did not (65%). However, overall contraceptive prevalence was about the same in both groups. Women who stop using oral contraceptives are more likely to be using traditional methods than women in the general population, especially if they want more children. Termination of pill use varies little according to the type of problem reported. Women with problems who sought medical attention were more likely to stop using the pill and so were women advised to stop by their physician, but the major factor affecting discontinuation was the reported experience of a problem.

One of the major activities of the printing press established by the late Emperor Haile Selassie in the early 1920s was the printing of religious works in Geez which were provided with an accompanying Amharic translation in keeping with the then Regent's known desire that these religious texts be made accessible to the public. This tradition has been continued by the Tənsa'e zäGvba'e and Täsfa Gäbrä-sәllase presses of Addis Ababa, at least until they were nationalized in 1976. The latter press has devoted most of its output to reprinting religious works in Geez and Amharic in cheap editions intended for sale to the pious lay public. Works in Geez may or may not be provided with an accompanying Amharic translation. Sometimes the Geez original and the Amharic translation are published separately. Occasionally the Geez text appears without an Amharic translation, and very rarely the Amharic text (as is the case with the version appearing below) appears without the Geez original. This is undoubtedly owing to the fact that the original Geez text or the Amharic translation were not available to the publisher.

Books dealing with the recent history of Ethiopia such as Greenfield's Ethiopia, a new political history, or Del Boca's The Ethiopian war, 1935–1941, state that a few works in Amharic have been written about this period. Though the number is not considerable, many books have been written on this subject, most of them by survivors of this conflict. Since they represent primary sources for the Ethiopian point of view, they are of great value, but there are several factors which complicate their utilization.

One of the commonplaces with regard to Ethiopian literature is the fact that it is heavily indebted to foreign sources for much of its material. The initial works of Ethiopic literature were made from Greek but as knowledge of that language faded, undoubtedly owing to the spread of Muslim hegemony which isolated Ethiopia from contact with Christianity outside Africa, translation from Greek came to an end. With the ‘restoration’ of the Solomonic dynasty in 1270, increased literary activity brought about the translation into Ge‘ez (Ethiopic) of a number of Arabic works, usually those of Christian writers.
